Senior Outreach & Case Management Services
The Senior Services Caseworker is responsible for providing confidential case management, outreach services and program coordination to senior citizens of the Town and their families . The Senior Services Caseworker will also serve as the Town’s Municipal Agent for the Elderly.
Senior Outreach
In conjunction with the Senior Center, there are several programs offered through Senior Outreach. Senior Outreach services include:
- As appropriate, home and/or office visits to provide an assessment of the resident's current health, financial and living situation
- Identification of and connection to community resources and funding options for home care services
- Developing an individualized Plan of Care based on the client’s needs/situation
- Maintaining a partnership with local organizations and community agencies, making referrals for senior residents as appropriate
- Ongoing outreach to Senior residents to encourage participation in activities and educational programs to promote socialization, well-being, and healthy lifestyles
Case Management
The Senior Services Caseworker provides confidential, ongoing case management to senior residents who would benefit from continuous support, guidance, and advocacy. This includes assessing individual needs, connecting seniors with essential services and resources, coordinating care plans, and providing assistance with benefits applications, housing, transportation, and social services to enhance their overall well-being and independence.
Case Management Services include:
- Helping residents gain access to support services such as
- Counseling
- Medical & Insurance
- Social, Educational & Financial Support
- Providing assessments, intakes, planning and implementation to achieve goals
- Assisting with the application process for the following programs: medical assistance programs as appropriate, food stamps(SNAP), Town and State social services, Social Security, protective services, housing and home care, adult day care transportation, and local senior and community groups.

Access Through Technology - The goal of the Access Through Technology program is to ensure that every person with combined hearing and vision loss has access to telecommunication equipment and necessary training, granting every individual the opportunity to interact and communicate with the world around them.
